State Subsidy Eligibility: Need Criteria

FranDelJA offers services for both private pay and subsidized childcare. Funding is offered through the California Department of Education, City Funding, and Alternative Payment Providers.

To apply for Private Pay or Subsidized Childcare for either of our FranDelJA locations, please contact us our administrative office, at (415)822-1699. We are happy to schedule an appointment for you to apply on campus, tour our facility, and discuss your child’s road to their future success.

STATE SUBSIDY ELIGIBILITY

NEED CRITERIA

To qualify for a subsidy, parents must verify need based on documentation of one of the following:
    1. Child Protective Services (Documentation required see Eligibility Criteria above)
    2. At-Risk (Documentation required see Eligibility Criteria above)
    3. Parental Incapacity: If you are incapacitated, you must provide:

    • A signed release statement authorizing a legally qualified health professional to disclose information necessary to establish that you meet the definition of incapacity and

    • A statement (form CD 3906) from a legally qualified health professional that includes the following:

      a. That the parent is incapacitated,

      b. that the parent is incapable of providing care and supervision for the child for part of the day,

      c. and the extent to which the parent is incapable of providing care and supervision,

      d. the days and hours per week that services are recommended,

      e. probable duration of incapacitation,

      f. name, business address, telephone number, professional license number, and

      g. signature of the health professional.

    6. Actively Seeking Employment
    7. Seeking Permanent Housing
    8. Or a combination of these for at least 32 hours per week
